Please read the following terms carefully before using Ambulance USA LLC dba Air Medical Transport (“AMT,” “our,” or “we”) website, www.AirMedical.com, as well as any online features, services, or programs offered by AMT (collectively, the “Website” or the “Site”).
YOUR USE OF THIS SITE IS SUBJECT TO THE TERMS AND CONDITIONS OF THIS AGREEMENT. BY ACCESSING, USING, OR VIEWING OUR SITE, YOU AGREE TO BE BOUND BY THESE TERMS OF USE, AND YOU AGREE TO THE TERMS OF AMT’S PRIVACY POLICY. IF YOU DO NOT AGREE, YOU MUST IMMEDIATELY EXIT THIS SITE.
By accessing or using the Website, you agree to the following terms. You should review these Terms regularly, as they may be updated or changed at any time at the sole discretion of AMT. Your continued use of the Website constitutes acceptance of such changes.
License and Website Access
AMT grants you a personal, non-exclusive, non-transferable, revocable license to access the Website for lawful purposes.
The Website may include software tools and databases that allow you to search for and retrieve information. You may not:
- Sell, distribute, or commercially exploit such tools, databases, or results;
- Use data mining, AI scraping, robots, or similar extraction tools;
- Sell, rent, lease, lend, or transfer any services provided on this Site.
You may not impersonate another person or entity, attempt to breach or compromise security, or disrupt the Website by flooding, probing, or using password-cracking programs. AMT reserves the right to terminate your access without notice and pursue all available legal remedies for violations.

Copyright, Trademarks & Trade Dress
All content included on the Website, including text, documents, forms, software, videos, images, sounds, names, logos, trademarks, service marks, and the compilation thereof, is the property of AMT, its affiliates, or its suppliers, and is protected by U.S. and international copyright laws.

All such marks are the exclusive property of AMT. Other trademarks or service marks displayed on the Website are the property of their respective owners.
Except as expressly authorized under “License and Website Access,” or as specifically permitted for transport-related documentation, quotes, or agreements, you may not:
- Copy, reproduce, publish, distribute, create derivative works of, sell, transfer, display, transmit, or compile into a database any copyright-protected element of the Website;
- Use any of AMT’s trademarks, logos, or trade dress without prior written consent;
- Utilize framing techniques to enclose any AMT trademark, logo, or proprietary content; or
- Use any metatags, hidden text, or similar code incorporating AMT’s trademarks or trade dress.
Credit Card Online Payment & Security
All credit card payments made online through our Website are processed via USAePay, a U.S.-based, Level-1 PCI DSS-compliant payment gateway.
Key protections include:
- TLS encryption (2048-bit) securing all transactions;
- Third-party security audits and compliance assessments;
- Fraud prevention controls such as duplicate-transaction detection and IP/country blocking;
- No cardholder data stored by AMT — payment information is encrypted, transmitted, and securely processed by USAePay.
By submitting payment through our Website’s Pay Online link, when you click on “Payment via Credit Card,” you acknowledge that your transaction is securely handled through USAePay and that AMT does not retain or have access to your full card details.
Content, Products, and Services
AMT reserves the right to modify, update, or discontinue any Website content or services at any time without notice. Third-party information may appear on the Site, and while AMT strives for accuracy, it does not guarantee completeness or reliability.

No Warranties; Indemnification
YOU EXPRESSLY AGREE THAT USE OF THE WEBSITE IS AT YOUR OWN RISK. THE WEBSITE, SERVICES, AND CONTENT ARE PROVIDED “AS IS” AND “AS AVAILABLE,” WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, EXCEPT TO THE EXTENT SUCH WARRANTIES CANNOT BE LEGALLY DISCLAIMED.
AMT provides the Website and the online service on a commercially reasonable basis. AMT does not guarantee uninterrupted, secure, or error-free operation of the Website or correction of defects.
The Client acknowledges and agrees that AMT service identifies, coordinates, and organizes air medical transportation from its worldwide aircraft network alliance of certified EASA and FAA Part 135 operators and providers. All logistics services are provided by independent third-party operators. AMT is not an operator. AMT and its affiliates do not guarantee the availability of transportation or logistics services, on-time arrivals or departures thereof, or any other service levels or performance related to the network of independent contractor providers obtained via the AMT service.
You agree to defend, indemnify, and hold harmless AMT, its affiliates, subsidiaries, owners, directors, officers, employees, and agents from any and all claims, demands, liabilities, damages, or expenses (including attorneys’ fees) arising out of your breach of these Terms or violation of third-party rights.
HIPAA & Confidentiality
AMT complies with HIPAA regulations through systems tailored for EMS and healthcare operations. Safeguards include provider agreements with contracted transportation operators, administrative controls, and secure technical systems.
Only the minimum necessary Protected Health Information (PHI) is collected for transport coordination. This may include the patient’s name, date of birth, medical diagnosis, required equipment, insurance details, and travel documentation.
By providing such information, the Client or patient’s authorized representative consents to AMT and its operators using PHI strictly for medical transportation, coordination, payment, insurance, and operational purposes.
AMT does not disclose PHI to unauthorized parties except as required by law or with prior written consent. All patient information and records are handled in compliance with U.S. federal and state HIPAA laws.
Limitation of Liability
IN NO EVENT SHALL AMT BE LIABLE FOR ANY DIRECT, INDIRECT, INCIDENTAL, SPECIAL, EXEMPLARY, PUNITIVE, OR CONSEQUENTIAL DAMAGES ARISING FROM YOUR USE OF THE WEBSITE OR RELATED SERVICES.
Where limitations are restricted by law, AMT’s liability shall not exceed the greater of (i) USD $100 or (ii) the amount paid by you for the specific service giving rise to the claim.
Waiver and Severability
No waiver by AMT of any term or condition shall be deemed a further waiver of such term or condition or a waiver of any other term or condition.
If any provision is found invalid or unenforceable, the remaining provisions shall remain in full effect.
Entire Agreement
These Terms of Use constitute the entire agreement between you and AMT regarding Website use and supersede all prior understandings or agreements related to this Website.
B) GENERAL TRANSPORTATION TERMS & CONDITIONS
The following are the general transportation terms and conditions for most medical transport types. Each type of transport has its specific terms and conditions, which differ for domestic and international transportation based on many variants. These variants include, but are not limited to, their unique complexity, type of transport, aircraft, flight team, logistics, duration, origin, and destination.
Before each transport, always refer to the individual Transport Contract you receive in writing for your specific booking request, as that document governs your Trip. Any modification to these Terms must be pre-approved and documented on your individual Transport Contract provided by AMT.
Ambulance USA LLC d/b/a Air Medical Transport (“AMT”) is a Patient Transportation Managed Organization that provides medical transportation arrangement services. AMT participates in identifying, organizing, and coordinating air medical transportation as per the requirements and stipulations of the respective Client.
For value received, AMT hereby agrees to arrange transportation for the Client. You (“the Client”), by accepting transport with us through our Website, whether by booking, paying online, or otherwise confirming services based on the information provided through www.AirMedical.com, are activating this “Trip,” accepting the terms & conditions of this Contract, accepting financial responsibility for all monies due and owed under this Contract including all services provided, and agreeing to be held fully responsible for all payments of monies owed.
You authorize AMT to charge your credit card or bank draft for the total price stated at the time of booking. You agree not to dispute those charges with AMT or your financial institutions. If AMT does not receive the total monies due or owed via the method and due dates described above, AMT reserves the right to convert the credit card authorizations listed above to charges for the collection of the balance due, including but not limited to cancellation fees or other costs incurred.
Transport Arrangement Authorization & Disclosure of Protected Health Information
By accepting transport through AMT, you authorize AMT to arrange your Trip and consent to the use and disclosure of the patient’s Protected Health Information for treatment, payment, and health care operations as permitted under HIPAA regulations.
AMT reserves the right, at its sole discretion, to postpone, modify, or cancel any Trip if medical clearance is not granted, is withdrawn, or if the patient’s condition changes from the information provided. In such cases, the Client remains fully responsible for all costs incurred up to the time of cancellation, including but not limited to repositioning, crew standby, and associated operational charges.
Pricing, Quotes, Discounts, and Additional Fees
All prices reflect a discount for pre-payment and a six percent (6%) discount for Wire or ACH. Payments by credit or debit card incur a six percent (6%) fee. Pricing is based on the programmed itinerary and anticipated charges.
- AMT will make commercially reasonable efforts to honor quoted terms if booking is approved on the same business day as the quote but does not guarantee original pricing if approved later.
- All quotes are valid for fifteen (15) days from issuance unless otherwise stated in writing, except for backhaul or empty-leg special discounts, which are valid only for the specified date.
- Additional costs may include, but are not limited to: de-icing, sea-level or low-level cabin surcharges (if medically required), war-risk surcharges, customs/immigration fees, after-hours airport operations, crew duty extensions, ground time over two (2) hours, visa clearance, operator or governmental surcharges, weather avoidance, holding, Air Traffic Control routing changes, parking fees, catering, hangar delays, or deviations/diversions beyond AMT’s control.
- A ten percent (10%) administrative fee applies to all third-party pass-through costs.
The Trip may be re-quoted if any part of the itinerary changes, and the Client shall pay any additional charges within forty-eight (48) hours of incurrence.
Availability of Solutions Provided
All solutions are provided globally on an availability basis at the time of booking. The Trip is subject to aircraft availability, flight team, overflight clearances, confirmation that the patient is Fit-to-Fly, and weather safety clearances.
You certify that all information you have provided is correct and current. In case of any change to the Trip or to the patient’s condition, additional charges may apply.
- All current medical records are required upon reservation.
- AMT and its operators reserve the right to decline the flight after reviewing the diagnosis and latest medical situation. If the patient’s condition is deemed too severe at the time of departure or does not correspond with the medical report provided, AMT reserves the right to refuse transportation in the interest of patient safety. In such cases, the full Service cost remains chargeable to the Client.
- AMT maintains the right to change, outsource, or subcontract the type of air and ground ambulance used depending on availability. Therefore, AMT is not responsible for their actions.
Travel Risks
The patient, purchaser, Client, patient’s legal guardian, next of kin, and any accompanying person(s) acknowledge and assume all risks inherent in air and ground transportation and agree to indemnify and hold harmless AMT, its officers, directors, employees, agents, contractors, medical personnel, and independent operators from any loss, damage, injury, or claim arising from delay, diversion, failure to depart or arrive as scheduled, or any cause beyond AMT’s control, including but not limited to Acts of God, weather, war, terrorism, riots, civil disorder, strikes, governmental actions, operator insolvency, mechanical or technical issues, or similar causes.
If diversion or an emergency occurs during flight, continuation of transport will be at the Client’s expense. The attending medical team and Pilot in Command may divert to the nearest suitable facility if medically required, and all related fees shall be borne by the Client. Trips may also require stops for fuel or crew changes due to duty time limits.
All crews, operators, and medical teams are independent contractors, and AMT is not responsible for their acts or omissions. AMT makes no guarantee or warranty regarding transport or medical results.
Limitation of Liability
To the maximum extent permitted by law, AMT, its partners, or affiliates will not be liable for any special, indirect, incidental, consequential, or punitive damages as a result of or in connection with the Client’s use of AMT’s services or any defect or fault resulting from the Client having breached the terms of this Contract.
In no event shall AMT’s total liability for any claim arising under or related to this Contract exceed a refund of amounts actually paid by the Client for the Trip in question, and such refund shall be the Client’s sole and exclusive remedy. This limitation does not apply in cases of AMT’s gross negligence or willful misconduct.
All air and ground operators, crews, and medical teams are independent contractors, and AMT is not responsible for their acts or omissions.
Passengers, Weights & Luggage
- One passenger and one patient, each weighing no more than 250 lbs. (113 kg), are permitted on board.
- Each passenger may carry one standard carry-on bag up to 35 lbs. (16 kg) [maximum 45 linear inches (22L x 14W x 9H in) or 113 linear centimeters (55L x 35W x 23H cm)] and one small personal item.
- Any excess luggage may be shipped separately at an additional cost.
Companion passengers require AMT pre-approval, and final authority for carriage rests solely with the Pilot in Command, taking into account safety, weight, balance, and medical considerations, including recommendations from the attending medical team.
AMT reserves the right to refuse, limit, or offload baggage without compensation if deemed necessary for safety or operational reasons. Any modification to these terms must be pre-approved and documented in your individual Transport Contract.
Bed-to-Bed Transportation
If your specific booking is requested with Bed-to-Bed transportation, ground transportation is included up to $500 per leg. If such price exceeds $500, the Client is responsible for the balance.
AMT will attempt to bill for reimbursement of the ground ambulance from your insurance provider. Any claims paid for ground transportation are non-refundable.
Cancellation
If the Trip is canceled for any reason by anyone other than AMT, the following applies, with “Dispatch Time” defined as the scheduled time the aircraft is dispatched to position for the Trip:
- Upon acceptance, a refund may be issued at AMT’s sole discretion, less $3,500 or ten percent (10%) of the flight cost, whichever is greater, plus any fees incurred up to the point of cancellation;
- If canceled less than thirty-six (36) hours prior to dispatch for domestic flights or less than seventy-two (72) hours for international flights, the Client will be charged fifty percent (50%) of the flight cost plus any fees incurred;
- Canceling after departure or failing to provide both oral and written notice with confirmed receipt by AMT will result in NO REFUND.
A Client-caused delay preventing dispatch or departure within two (2) hours of the scheduled dispatch time will be deemed a same-day cancellation and subject to the applicable cancellation fee.
All cancellation fees are in addition to costs already incurred, and refunds exclude credit card and bank fees.
Payment Terms and Enforcement
- Full cleared payment in U.S. dollars (USD) is required prior to aircraft dispatch.
- AMT may suspend or cancel the Trip without liability if payment is not received by the required deadline.
- Should any amount remain unpaid over thirty (30) days, AMT reserves the right to bill this Contract at the usual and customary rate of $30,000 base rate plus $180 per mile, together with any attorney fees, collection fees, and interest to the maximum permissible by law, all at the expense of the Client, without prejudice to any other remedies available to AMT.
Additional Terms
Binding Effect: All rights, obligations, and liabilities under this Contract are binding on the respective agents, heirs, executors, administrators, and successors of the parties. If the Trip is booked by a broker, agent, or intermediary, both such party and the ultimate Client shall be jointly and severally liable for all obligations under this Contract.
Entire Contract: This Contract constitutes the entire agreement between the parties, and all prior or contemporaneous oral or written statements are merged and superseded herein.
Applicable Law, Dispute Resolution, Jurisdiction, and Venue: This Contract shall be governed, interpreted, and construed under the laws of the State of Texas, USA, without regard to its conflict-of-law principles. Before initiating legal action, the Client must contact AMT and allow AMT an opportunity to resolve the matter. If resolution is not reached, the parties will engage in good faith negotiation, then mediation in Fort Bend County, Texas, USA. If mediation fails, disputes shall be resolved by binding arbitration in Fort Bend County under the AAA Commercial Arbitration Rules, conducted by a single arbitrator with substantial experience in aviation and medical transport law, with costs shared equally. The Prevailing Party shall be entitled to recover all reasonable attorneys’ fees, expert witness fees, and costs. All parties consent to jurisdiction and venue in said forum.
Modification: Any modification or amendment must be in writing and signed by all parties. Any special requirement or medical condition must be stated in this Contract before each Trip; failure to do so releases AMT from liability.
Severability: If any provision is declared invalid or unenforceable by a court of competent jurisdiction, it shall not affect the validity of the remaining provisions.
