- Completed Reservation Form, including all passenger details and sending/receiving facility contacts.
- Updated medical report, detailing the patient’s current condition, vital signs, onboard medical-equipment needs, and treatment plan during flight.
- For international transports, clear passport and visa copies (if applicable) for the patient and accompanying companion.
